16 Metro Phoenix Restaurants Scored a 'D' on County Health Inspections in April

Every week, the county's health inspectors within Maricopa County Environmental Services give restaurants grades after completing inspections. Participation in the rating system, however, is voluntary, which means owners or managers do not have to accept a grade after an inspection; instead, the restaurants are listed online as "not participating." And in some cases, grades posted online may change, which typically means a restaurant has provided necessary documentation for food safety or fixed a critical issue that led to a low grade. Here's a more detailed explanation.

A few words that stood out out in last month's county health inspections: "slime," "cockroaches," "pants," and "deteriorating." April was another standout month for inspection violations, with 16 Valley spots racking up enough infractions to warrant a 'D' grade from inspectors.

Prankster's Too, Scottsdale (7919 East Thomas Road, Suite 101)

"Raw chicken in a flat pan observed placed above cooked chicken inside the walk in cooler. The food wrap used for the raw chicken was observed resting on the cooked chicken. PIC discarded the cooked chicken as there were signs of contamination."

Super Carniceria Uruapan No 2, Glendale (6541 North 59th Avenue)

"In the walk-in cooler TCS food items such as chimichanas, green salsa, ceviche, cooked ribs, cooked beef feet, guacamole, pasta, and beef tacos did not have date marking. Per person in charge (PIC) the food items were cooked within the previous two days and normally are held for about 3 days."

Zipps Sports Grill, Phoenix (4710 East Warner Road)

"Observed employee exit building and return to slicing sweet potatoes, without washing hands. Requested employee to hand wash in between task change to reduce food born illness."

Great Papa's Gyros LLC, Glendale (4935 West Glendale Avenue, Suite 4)

"Machine for cooking gyro meat is broken and does not turn. Person in charge removed gyro meat. Spoke with PIC about methods that can be used for gyro meat until machine is fixed."

Chick-Fil-A, Phoenix (3001 West Agua Fria Freeway)

"Staff handling raw chicken with yellow gloves and then removing gloves and putting on clear gloves to handle ready-to-eat chicken. Staff had visible flour breading mixture on lower arms. Instructed staff to immediately remove gloves and wash hands. Discussed when to wash requirements after handling raw chicken to avoid the potential for cross contamination."

Brookdale North Chandler, Chandler (Assisted living, 2555 North Price Road)

"During the time of the inspection 2 employees washed their hands without soap at the hand wash sink in the kitchen. The employees rewashed their hands with soap upon request."

Panaderia Y Tortillera Guerrero, Phoenix (2829 North 32nd Street, Suite 103)

"Live baby cockroaches found near hand wash sink. Obtain certified pest control records before re-inspection in 10 days."

U.S. Egg, Phoenix (2957 West Bell Road)

"Two large knives stored as clean had food debris on the surface and food debris build-up on bottom of large tub where attachments and blades to large food processor are stored as clean. Black material build-up on interior surface of ice machine not in contact with ice. Instructed manager on proper disassembly of equipment for proper cleaning with manager."

Menuderia Guanajuato, Phoenix (124 South 24th Street)

"Dead cockroaches (approximately 10) found in 2 dry storage areas underneath shelving units. Sticky traps were also full and needing replacement. Ensure dead pests are removed of. Employee swept them up at time of inspection and threw them away."

Boca Taqueria, Gilbert (1674 North Higley Road, Suite 101)

"Observed bare hand contact with ready to eat tacos to handle toppings (salsa, cheese and lettuce) and burritos by the cooks at the time of the inspection. Informed person in charge and cooks that bare hand contact with ready to eat foods is prohibited at all times."

Chipotle Mexican Grill, Phoenix (3009 West Agua Fria Freeway, Suite 1)

"Large cut lettuce spinner and basket had lettuce debris on the surface. Per manager, cut lettuce and prepared over 4 hours prior to inspection. Discussed cleaning frequency, clean to sight and touch and 3 step in-place cleaning process. Two white cutting boards stored as clean had food debris build-up on the surface. Manager will have staff properly clean equipment at time of inspection."

Roliberto's Mexican Food No 2, Phoenix (3414 West Union Hills Drive, Suite 2)

"Two plastic spatulas had missing pieces at ends and large white cutting board had heavy black material build-up on the surface. Metal blade attachment and metal and black plastic attachment to hand chopper was rusted with debris build-up on the surface. Maintain all equipment smooth and easily cleanable."

Lin's Chinese Buffet, Phoenix (7406 West Mcdowell Road)

"Ice Machines in both drink stations had the presence of slime mold. Both machines are wiped down with sanitizer at time of inspection."

Hooters, Phoenix (10223 North Metro Parkway)

"Walls to entrance to kitchen deteriorating and a couple of walls in kitchen have holes, floor tiles missing in walk-in cooler at entrance to walk-in freezer. Repair and maintain smooth and easily cleanable."

Filiberto's Mexican Food, Mesa (531 East Southern Avenue)

"Observed employee grab gloves from pocket of pants and put on gloves to handle food. Instructed employee to remove gloves and put on new pair from box of gloves."

The Stratford, Phoenix (Assisted living, 1739 West Myrtle Avenue)

"Multiple flies throughout kitchen, ware wash, and storage areas. Ensure that pest control is performed."
