Every week, the county's health inspectors within Maricopa County Environmental Services give restaurants grades after completing inspections. Participation in the rating system, however, is voluntary, which means owners or managers do not have to accept a grade after an inspection; instead, the restaurants are listed online as "not participating." And in some cases, grades posted online may change, which typically means a restaurant has provided necessary documentation for food safety or fixed a critical issue that led to a low grade. Here's a more detailed explanation.

It was not a good month for Filiberto’s. Three locations of the restaurant showed up on May’s D list — earning a multitude of health-inspection violations. Local favorites like Mucha Lucha Taco Shop and Bosa Donuts also earned D grades.

Bosa Donuts (3722 East Indian School Road)

“Observed two large bags of powdered glaze mix soaked through and laying in a puddle of brown liquid that was identified as iced coffee by the person in charge.”

Fry's Food Store #40 (3421 West Thunderbird Road)

“Spray bottle of chemical cleaner stored on food prep table next to two open packages of ready-to-eat deli meats, large tub of ready-to-eat cut lettuce and sliced hoagie rolls.”

Friendship Village Of Tempe (2645 East Southern Avenue, Tempe)

“Raw chicken stored behind pork in drawer in cookline in kitchen. Raw turkey stored behind raw beef in cafe area. Items were moved by person in charge. Discussed proper storage of raw food items.”

Poliberto’s (1219 West Baseline Road, Tempe)

“The ceiling of the ice machine found to contain accumulation of matter. Please clean inside of ice machine to prevent contamination.”

Filiberto's Mexican Food (10227 North Metro Parkway)

“Large dead black cockroach in hand washing sink basin and several dead roaches on floor under shelving units by 3 compartment sink. Remove all dead roaches to maintain facility in good sanitary condition.”

Giant Rustic Pizza (3945 East Camelback Road)

“Observed tomato sauce hot held to 116 F. Sauce was voluntarily discarded. Hot held items shall be kept at 135 F.”

Willie’s Taco Joint (333 East Jefferson Street, #102)

“Observed employee use a bare hand to attempt place lime wedge on beverage. Had employee discard lime wedge and re-garnish drink using tongs. Reviewed use of utensils or gloves to handle ready to eat foods including garnishes.”

Doubletree Suites (320 North 44th Street)

“Observed employee use bare hands to place cucumber on plate and also use bare hands to keep fried food from falling off of plate. Items were discarded and employee was educated about not using bare hands on ready to eat foods.”

Carniceria El Herradero (101 West Frye Road, Chandler)

“Raw pork skins and stomachs found stored above cooked chicharrones in walk-in refrigerator in outside area. Chicharrones were moved over so that they were no longer under raw pork.”

IHOP #1513 (2501 North 44th Street)

“Employee handling/cracking raw eggs went to handle clean equipment without changing gloves and washing hands.”

Teharu Sushi (9845 South Priest Drive, #106, Tempe)

“Observed food debris on line that contain revolving sushi plates. Please clean and maintain to prevent food particle build up on line.”

Jimmy Johns (2170 North Arizona Avenue, Chandler)

“Observed various deli meats above fill line in the top part of the sandwich prep table from 47F-53F. Had person in charge remove meats to obtain correct fill level in prep unit. Discussed proper cold holding temps.”

Filiberto's #58 (5220 West Baseline Road, Laveen)

“Scoop in rice bin was improperly stored with handle touching the rice. Scoop was corrected. Store scoops with handle away from food.”

Filiberto's Mexican Food (1601 East McDowell Road)

“Outdoor garbage area has large accumulation of debris outside of dumpster. Clean area, and maintain clean.”

Mucha Lucha Taco Shop (7620 East McKellips Road, Scottsdale)

“Observed pork on hot holding with an internal temperature of 127 F. The manager indicated that the pork had been at the steamer for 1hr. Instructed the manager to reheat the pork to 165 F and then hot hold at 135 F. Always ensure that TCS food items for hot holding are maintained at 135 F or above.”
